Mengisahkan dua orang remaja yang lahir dan hidup ditempat yang berbeda. Mitsuha adalah seorang gadis SMA yang tinggal di daerah pedesaan di pegunungan. Ayahnya yang jarang pulang kerumah membuat Mitsuha hanya hidup bersama nenek dan adiknya yang masih SD. Mitsuha sering berkeinginan untuk pergi ke Tokyo, karena bosan hidup di pedesaan. Sementara itu, Taki adalah seorang siswa SMA di kota Tokyo. Dalam kesehariannya, ia menghabiskan waktu untuk bermain dengan teman-temannya dan bekerja paruh waktu di restoran Italia. Dia juga tertarik dengan kesenian dan arsitektur. Suatu hari mereka mengalami pengalaman aneh dimana Mitsuha bermimpi dirinya menjadi seorang laki-laki.
